【摘要】 目的 探討阿加曲班對大鼠肢體缺血再灌注損傷對血液流變學(xué)指標(biāo)的影響,為缺血再灌注損傷的防治提供新思路。方法 采用大鼠肢體缺血再灌注模型,將14只Wistar大鼠隨機(jī)分為2組,對照組和阿加曲班組,每組7只。2組大鼠均于再灌注2 h后,酶聯(lián)免疫吸附(ELISA)法測定血漿P選擇素、細(xì)胞間粘附分子1(ICAM1)濃度;取血檢測血液流變學(xué)指標(biāo),包括血漿粘度(ηp)、全血低切還原粘度(Lηre)、全血高切還原粘度(Hηre)、紅細(xì)胞壓積(HCT)、紅細(xì)胞聚集指數(shù)(EAI)、血沉方程K值(ESRK)、紅細(xì)胞剛性指數(shù)(TK)、紅細(xì)胞變形指數(shù)(EDI)。結(jié)果 大鼠肢體缺血再灌注2 h后,阿加曲班組血漿P選擇素濃度、血漿ICAM1濃度顯著低于對照組(P<0.01)。血液流變學(xué)指標(biāo)與對照組比較,阿加曲班組ηp、Hηre、Lηre、EAI和TK明顯降低(P<0.05),EDI升高(P<0.05),而對Hct、ESRK無顯著影響(P<0.05)。結(jié)論 阿加曲班能夠降低血漿P選擇素和ICAM1濃度,進(jìn)而改善大鼠肢體缺血再灌注損傷所造成的血液流變學(xué)異常。

【Abstract】 Objective To investigate the effects of agatroban on hemorheological parameters,plasma Pselectin and plasma ICAM1 of rats with limb ischemia/reperfusion injury in order to provide a new method for the prevention and therapy of ischemia/reperfusion injury. Methods 14 Wistar rats were randomly divided into control group and agatroban group, with 7 rats in each group.The rat model of limb ischemia/reperfusion injury was made according to the ZHANG Lianyuan’s ways. The plasma Pselectin and plasma ICAM1 levels were detected by enzyme linked immunosorbent assay (ELISA) at 2h after reperfusion. The blood samples were taken and hemorheological indexes were detected, including plasma viscosity, low shear reduction viscosity, high shear reduction viscosity, hematocrit erythrocyte aggregation index, erythrocyte sedimentation rate K and erythrocyte rigidity index.Results 2h after limb ischemia/reperfusionin rats,the plasma Pselectin and plasma ICAM1 levels in argatroban group were significantly lower than those of control group(P<0.05).As compared with those in control group,the ηp、Hηre、Lηre、EAI and Tk in argatroban group were significantly decreased(P<0.05),however,EDI was obviously increased(P<0.05),but no obvious effect on Hct,ESRK was observed.Conclusion Agatroban can reduce plasma Pselectin and plasma ICAM1 levels,as a result,to improve hemorheological abnormality caused by limb ischemiareperfusion injury in rats.

近年隨著對缺血再灌注損傷基礎(chǔ)研究的深入,對其再灌注損傷的保護(hù)研究也越來越多。試圖應(yīng)用不同的方法,通過不同的途徑發(fā)揮作用。缺血預(yù)處理、抗氧化療法、抗白細(xì)胞療法等,都有很好的保護(hù)作用[1,2]。阿加曲班是化學(xué)合成的低分子藥物,直接與凝血酶的催化活性位點(diǎn)結(jié)合,滅活凝血酶。近年研究表明阿加曲班還可抑制凝血酶的其他作用,如可以抑制腫瘤轉(zhuǎn)移、炎性過程及血管成形術(shù)后的再狹窄等[3],對微循環(huán)障礙和血液流變性也有一定影響[4]。但能否對肢體缺血再灌注損傷導(dǎo)致的血液流變學(xué)異常有良好的改善作用,我們對大鼠肢體缺血再灌注損傷進(jìn)行了實(shí)驗(yàn)研究,觀察對比阿加曲班干預(yù)和單純?nèi)毖俟嘧p傷導(dǎo)致的血液流變學(xué)異常有無顯著性影響,為缺血再灌注損傷的臨床防治提供實(shí)驗(yàn)基礎(chǔ)醫(yī).學(xué)全.在.線網(wǎng)站f1411.cn。
